What to Expect at Your First Visit and More

Before your first visit you will need to complete our registration forms. Please download and print our new patient forms using the links below. We ask that you please complete all forms in advance so that we may better serve your needs.

At your first visit, we will review your medical and dental history and perform a comprehensive examination. The examination will include an oral cancer screening, cavity detection, periodontal (gum) exam, evaluation of the bite and screening for jaw joint problems. We will do a cosmetic evaluation and discuss your concerns and desires related to your dental appearance. X-rays and study casts of your mouth, if indicated, will be taken at this time. If treatment needs are not complex, we will discuss our findings and possible solution options for you at this visit. For more complex treatment recommendations, we may ask you to reschedule for a second consult visit with the doctor.

Financial Information

Our goal is to provide the best possible dental care for each person in a way which respects the individual needs of the friends we call patients. Clear financial arrangements play an important role in our ability to allow our patients to attain good dental health. The following financial options are offered.

  • Payment at the Time of Service: We accept cash, checks or credit cards at the time of the appointment. If treatment will extend for several visits, you may pay a portion of the total fee at each visit, with payment-in-full on the last visit.
  • Extended Financing Option: As a service to our patients, we are pleased to offer CareCredit (www.carecredit.com), the nation’s leading dental payment plan. CareCredit has several payment plans to meet every financial need, including interest-free and low-interest options, each with low monthly payments and no upfront costs. It’s easy to apply and you will receive an online decision in seconds. Apply online or speak to our office manager for more details.

Patients with Insurance Coverage

Dental insurance pays only a portion of the total treatment charge. The portion that the patient pays (Deductible and Co-Payment) can be handled according to the Payment Options outlined above. Our office will gladly file all necessary claims on the patient’s behalf. Certain insurance companies will not send payments to our office but will send them to patients. In those situations, patients will pay for their services according to the options above and will be reimbursed directly by their carriers to the extent they allow.

Patients are always responsible for keeping their account current regardless of any dental insurance coverage.

The staff at Chesheim Dental are extremely friendly and professional. The office is very warm and inviting and we are always seen within 5-10 minutes of our appointment time. My son, who has autism, was terrified of the dentist and the hygienists and Dr. Angel are wonderful with him. In fact, he no longer fights me when he has a dental appointment and the Chesheim staff make what could be a difficult experience for him very tolerable.
